Health Care Plan (HCP)
Who needs one of these plans?
Any pupil who has a medical condition which needs to be considered when planning for their education.
Any pupil who needs any regular or acute medical intervention.
Who writes it?
The lead in writing this plan is the school nurse, in liaison with you. The nurse will also talk to your child’s paediatrician and other specialist medical professionals involved in your child’s health and class staff will support with this process.
It is updated as required and reviewed annually.
Why is the plan there?
The plan is there to ensure all staff in school deal appropriately with your child’s health care needs, including staff having the right training. It is also considered when putting together the education for your child so that whatever health care is needed fits seamlessly into their school day.
What is the outcome of having a plan?
By having a HCP we ensure everyone who needs to be, is aware of current health care needs and can act appropriately to support your child so they can learn to their best ability.
Eating and Drinking Plan
Who needs one of these plans?
Anyone who needs support in developing eating and drinking skills.
Who writes it?
The lead in writing this plan for dysphagia is the speech and language therapist. Staff will liaise with you when writing it. Eating and drinking skills development programmes are written by class leads with support from health care professionals i.e. Occupational therapists.
Why is the plan there?
Having a plan ensures your child is safe and has consistency in how they are supported with eating and drinking with regard to equipment, textures of food and seating position.
What is the outcome of having a plan?
Creating the best and safe practice to support developing eating and drinking skills.
Moving and Handling Plan/ MOVE programme
Who needs one of these plans?
Anyone who needs support with their mobility and developing gross motor skills.
Who writes it?
The lead for this plan is the school’s manual handling instructor, who will seek support from class staff, physiotherapists, occupational therapists and LA’s Moving and Handling advisor if needed. It is updated as required and reviewed annually, but always when new equipment or slings are introduced and if there is any change in a pupil’s physical needs.
Why is the plan there?
The plan sets out how best to move and handle your child. This could be supporting walking or support in moving and out of various equipment such as walkers, standers, side-lyers. Not all specific situations may be described, but the principles of how to move a pupil is covered.
What is the outcome of having a plan?
By having this plan we ensure all staff are trained in the specific technique of supporting a pupil’s mobility and can do this so your child is as safe and comfortable as possible and ready for learning.
Behaviour Support Plan (BSP)
Who needs one of these plans?
Any pupil who would benefit from a structured approach to support their behaviours to enable them to engage in learning.
Who writes it?
The class lead writes this with support from the school welfare team and family. Sometimes it may be beneficial to seek the support of the educational psychologist or Child and Adolescent Mental Health Service. It is reviewed as required.
Why is the plan there?
All staff are trained in positive behaviour support strategies. The BSP sets out what behaviours are displayed and in what situations they occur. It also sets out how the team best support the pupils in order to create an environment which supports the pupil to self-regulate and re-engage in learning.
What is the outcome of having a plan?
By having a BSP everyone around the pupil will know how to create the best environment for pupils to optimise their learning.
